Personally,I think Twitter's problem is the people running it are dependent on users being harassed and trolled to self police and block the attackers themselves.But besides a dedicated security team,I really don't know what Twitter could enforce that wouldn't fundamentally break what the social media platform was created for. That in my opinion is open-ness.
